Brief History of the State of the Union Address The State of the Union address is a speech delivered annually by the President the United States to a joint session of the United States Congress. The State of the Union Address is not, however, delivered during the first year of a new president’s first term in office. In the address, the president typically reports on the general condition of the nation in the areas of domestic and foreign policy issues and outlines his or her legislative platform and national priorities. Delivery of the State of the Union address fulfills Article II, Sec. 3, of the U.S. Constitution requiring that â€Å"The President shall from time to time give to Congress information of the State of the Union and recommend to their Consideration such measures as he shall judge necessary and expedient.†Ã‚   As a policy of the doctrine of separation of powers, the Speaker of the House must invite the president to present the State of the Union Address in person. In lieu of an invitation, the address can be delivered to Congress in written form. Since January 8, 1790, when George Washington personally delivered the first annual message to Congress, presidents have from time to time, been doing just that in what has become known as the State of the Union Address. The speech was shared with the public only through newspapers until 1923 when President Calvin Coolidges annual message was broadcast on radio. Franklin D. Since the non-publicized trip included Afghanistan- an active war zone- travel had been arranged aboard a U.S. Air Force plane. Trump had earlier canceled his own upcoming trip to the World Economic Forum in Davos, Switzerland, due to the shutdown. On January 23, President Trump turned down Speaker Pelosi’s request to delay his State of the Union Address. In a letter to Pelosi, Trump asserted his intention to deliver the address on Tuesday, January 29 in the House chamber as originally scheduled. â€Å"I will be honoring your invitation, and fulfilling my Constitutional duty, to deliver important information to the people and Congress of the United States of America regarding the State of our Union,† Trump wrote. â€Å"I look forward to seeing you on the evening on January 29th in the Chamber of the House of Representatives,† he continued, adding, â€Å"It would be so very sad for our Country if the State of the Union were not delivered on time, on schedule, and very importantly, on location!† Speaker Pelosi has the option of blocking Trump by refusing to call a vote on the resolution required to formally invite the president to before a joint session of Congress in the House chamber. Lawmakers have not yet considered such a resolution, an action typically taken for granted.   Speaker Pelosi quickly returned this historic struggle of separation of powers back to where it started on January 16 by informing President Trump that she would not allow him to deliver his speech in the House chamber as long as the government shutdown continued. President Trump responded by indicating that he would announce plans for an alternative State of the Union address at a later date. A White House spokesperson suggested options including a speech from the Oval Office of the White House or at a Trump rally away from Washington. In a late night tweet on January 23, President Trump conceded to Speaker Pelosi, stating that he would delay his State of the Union Address until after the government shutdown had ended. â€Å"As the Shutdown was going on, Nancy Pelosi asked me to give the State of the Union Address. I agreed. She then changed her mind because of the Shutdown, suggesting a later date. This is her prerogative- I will do the Address when the Shutdown is over,† Trump tweeted, adding, â€Å"I look forward to giving a great State of the Union Address in the near future!† The President continued that he would not seek an alternative location for the annual speech â€Å"because there is no venue that can compete with the history, tradition and importance of the House Chamber.† In a tweet of her own, Speaker Pelosi said she was hopeful that President Trump’s concession meant that he would back a bill already before the House that would temporarily fund the federal agencies affected by the shutdown. On Friday January 25, President Trump reached an agreement with Democrats on a short-term spending bill that did not include any funding for the border wall but allowed the government to temporarily reopen until February 15. During the delay, negotiations over border wall funding were to continue, with President Trump stressing that unless funding for the wall was included in the final budget bill, he would either allow the government shutdown to resume or declare a national emergency allowing him to reallocate existing fund for the purpose. On Monday, January 28, with the shutdown at least temporarily ended, Speaker Pelosi invited President Trump to give his State of the Union address on February 5 in the House Chamber. â€Å"When I wrote to you on January 23rd, I stated that we should work together to find a mutually agreeable date when government has reopened to schedule this years State of the Union address,† Pelosi stated in a letter provided by her office. â€Å"Therefore, I invite you to deliver your State of the Union address before a Joint Session of Congress on February 5, 2019 in the House Chamber.† President Trump accepted Pelosi’s invitation a few hours later. The Address At Last President Trump finally delivered his second State of the Union address on February 5th in the House Chamber. Definition of Gadsden Purchase The Gadsden Purchase was a strip of territory the United States purchased from Mexico following negotiations in 1853. The land  was purchased because it was considered to be a good route for a railroad across the Southwest to California. The land comprising the Gadsden Purchase is in southern Arizona and the southwestern part of New Mexico. The Gadsden Purchase represented the last parcel of land acquired by the United States to complete the 48 mainland states. The transaction with Mexico was controversial, and it intensified the simmering conflict over slavery and helped to inflame the regional differences that eventually led to the Civil War. Background of the Gadsden Purchase Following the Mexican War, the boundary between Mexico and the United States set by the 1848 Treaty of Guadalupe Hidalgo ran along the Gila River. Land to the south of the river would be Mexican territory. When Franklin Pierce became president of the United States in 1853, he backed the idea of a railroad that would run from the American South to the West Coast. And it became apparent that the best route for such a railroad would run through northern Mexico. The land to the north of the Gila River, in United States territory, was too mountainous. President Pierce instructed the American minister to Mexico, James Gadsden, to purchase as much territory in northern Mexico as possible. Pierces secretary of war, Jefferson Davis, who would later be the president of the Confederate States of America, was a strong supporter of a southern rail route to the West Coast. Gadsden, who had worked as a railroad executive in South Carolina, was encouraged to spend up to $50 million to buy as much as 250,000 square miles. Senators from the North suspected that Pierce and his allies had motives beyond simply building a railroad. There were suspicions that the real reason for the land purchase was to add territory in which slavery could be legal. Consequences of the Gadsden Purchase Because of objections of suspicious northern legislators, the Gadsden Purchase was scaled back from the original vision of President Pierce. This was an unusual circumstance where the United States could have obtained more territory but chose not to. Ultimately, Gadsden reached an agreement with Mexico to purchase about 30,000 square miles for $10 million. The treaty between the United States and Mexico was signed by James Gadsden on December 30, 1853, in Mexico City. And the treaty was ratified by the U.S. Senate in June 1854. The controversy over the Gadsden Purchase prevented the Pierce administration from adding any more territory to the United States. So the land acquired in 1854 essentially completed the 48 states of the mainland. Incidentally, the proposed southern rail route through the rough territory of the Gadsden Purchase was partly the inspiration for the U.S. Army to experiment by using camels. The secretary of war and proponent of the southern railway, Jefferson Davis, arranged for the military to obtain camels in the Middle East and ship them to Texas. It was believed the camels would eventually be used to map and explore the region of the newly acquired territory. Following the Gadsden Purchase, the powerful senator from Illinois, Stephen A. Douglas, wanted to organize territories through which a more northern railroad could run to the West Coast. And the political maneuvering of Douglas eventually led to the Kansas-Nebraska Act, which further intensified tensions over slavery. If ones thoughts are good they he/she will see good dreams and if someone’s thoughts are bad they he/she will se bad dreams. From the above line it is clear that dreams are good or bad, intentions of people. All of us dream, several times at night. It is believed by some that we sleep in order that we may dream. Dreams can come true if somebody makes them true, as the saying goes, â€Å"A dream is just a dream, unless you make it come true†. Dreams provide us the actual picture of our thoughts. Dreams may tell us about any physical event which took place with us or which is going to happen with us. The dream is trying to inform the dreamer about his condition in any walk of life. Basically, we can dream about anything logical or illogical, fictious or non-fictious and reasonable or unreasonable. Dreams can often give some people good ideas, which they could not imagine. It is said that Robert Louis Stevenson got his idea for Dr. Jekyll and Mr. Hyde from a dream. But some people think that they are not able to apply these ideas in their life so there is a well-known saying about them which says that: â€Å"IF YOU CAN DREAM IT, YOU CAN DO IT†. Mean to say that whatever one watches in dream he/she should try to implement it in his/her personal life and if that idea is good one that one should also pass it to others and also ask them to implement it on their lives as well. In somebody’s darkest or boring hour, dream can give somebody hope, courage and determination to some creative work which somebody has not done before. All successful people are big dreamers. They imagine what their future could be ideal or bad. Generally, everybody in this world dreams, but everybody has not aggressiveness to work out on the ideas that they got in their dream.

Policy Priority Issue Those who utilize the Medicaid system range from low income families to the over 65 age group. Within this population is also those who are disabled due to physical or mental problems. This is among the sickliest of our American population. A paper based on a study in Oregon stated that â€Å"Medicaid significantly increased the probability of being diagnosed with diabetes, and being on diabetes medication as well as high blood pressure and high cholesterol.†(Baicker et al., 2013, p. 1715). Much of this is due to the struggle that the Medicaid beneficiary has to accessing all of the benefits of the program. This mostly consists of medical appointments, especially those related to a specialist physician. This paper will identify the importance to make available proper healthcare to those who receive Medicaid. Not only with specialty doctors but also with their own primary physicians. The need to transfer the ownership from the government to the patient is necessary for better healthcare outcomes. This is directly correlated to the care received and expected by the patient. This will result in a better outcome both medically for the patient and fiscally for the government. The specific problem Medicaid is a government sponsored and run program, it provides care to over 53 million low income Americans yearly and has an average operating budget of $349 billion dollars (O’Shea, 2007). This increasingly high expenditure has caused much strained to the budgets of not only the federal government but also the state budgets as well. With the changes to the way we view healthcare and the use of evidence based practice in the care delivered it is evident that the quality of care given to those with Medicaid is missing. Evidence has shown that patient’s with Medicaid receive inferior care than those with private insurance based solely on the access they have to the Medical providers, especially those who provide specialty care (O’Shea, 2007). In a survey conducted in 2003, it highlighted that the recurrent problem is the reimbursement rate from Medicaid to the physician (O’Shea, 2007). The Center for Studying Health System Change (HSC) show that 21% of physicians that state they accept Medicaid have reported they will not accept a new Medicaid patient in 2004-2005(O’Shea, 2007). This number would only logically be assumed to have risen in 2013 A survey conducted by the U.S. National Health reported that researchers have found two standout trends among Medicaid beneficiaries: they have more difficulty getting primary care and specialty care and they visit hospital emergency departments more often than those with private insurance (Seaberg, 2012). The lack of primary and specialty care access is mostly contributed to the following barriers; unable to reach the MD by phone, not having a timely appointment with the MD and lastly unable to find a specialty MD that will accept Medicaid. In a recent report released by the Partnership to Fight Chronic Disease, it stated that about 30% of Medicaid patients experience â€Å"extreme uncoordinated care†, there is a strong correlation between this situation and higher Medicaid spending and less quality of care given (Bush, 2012). After January 1st 2013, healthcare providers have experienced a 2% reduction in payments for Medicaid beneficiary, this will only create more of a problem for these patients to seek the care thy desperately need. The question must be asked, how can we give the care necessary to those with Medicaid and also make this as cost effective for the providers seeing the patient as well. Steps to Change There is much conversation in government today that would expand the Medicaid program, but there is no discussion on payment to physicians, hospitals and other providers, which is the main problem at hand. A system that would most be beneficial is one that is centered on the patient-doctor relationship (Felland, Lechner, & Sommers, 2013). This would not only improve the standard of care given to the patient but help with the fiscal decline related to a very broken system that is presently in place. Our aim should be to stop the decision making from the government, and transfer this power to the individual as well as promote individual responsibility for healthcare choices. A policy that would restructure the financing of healthcare to assist low-income families and medically needy Americans to purchase coverage that would best meet their needs and their medical situations. This approach would need to be Nationwide and would be a great undertaking to accomplish. The rational approach model would be the model of choice for this type of change. It would take many years to accomplish based on financial barrier as it pertains to the various budgets at the state and federal levels. However, the end result would be that a personal ownership of health insurance, and control over the flow of dollars in the health care system, this will enhance personal responsibility. Another aspect it would accomplish would be that patients would demand and receive better value for health their care dollars. This ownership of their own care would raise much awareness in the patients and facilitate many quality conversations with their healthcare provider. This is an opportunity currently unavailable to patients enrolled in the Medicaid program and has resulted in a system that does not give quality care and the costly price to the government is evident. Until we can have all 50 states participating in this policy change Nationwide, there are some steps that can be taken with the present system in place. These would involve the use of technology in interesting situations. If you cannot get the patient to the physician, regardless of primary or specialty, bring the physician to the patient. Using telehealth programs would help identify problem before they are in need of emergency attention (Felland et al., 2013). Kentucky used this system to help with the overpopulated Medicaid system and this state was able to save money and provide quality care to those who are in need of it (Ungar, 2013). Another solution would be to expand the primary role of the primary provider. Utilizing the Advanced Registered Nurse Practitioner (ARNP) to take on the care of patients in the role of the Primary Provider would help ease the burden for the dwindling Medical Community (Felland et al., 2013). With only 7% of the graduating physicians choosing primary care a large strain is evident on the medical community to provide care to patients (Sellers, 2013). Both the above policy changes would best be suited under the stage-sequential model. Putting both of these changes in to place at the same time in multiple areas then re-evaluating the need for changes in order to define a process that could be utilized Nationwide would be important for success. Both of these policy changes would give way to better care for the Medicaid beneficiary, better outcomes and a use of funds that would be cost effective as well. Conclusion In conclusion this paper has reviewed the importance of change needed to the present Medicaid system. The present system has shown to take away the ownership of healthcare from the patient and given it to the government. In doing so several areas for needed improvement have been identifies; cost, lack of resources among physicians and most important below quality care. All resulting in a system that is not able to deliver quality care to those who are in desperate need of it the most. By implementing the changes outlined in this paper, it will be able to change and improve these barriers for the better, resulting in better patient care outcomes, better financial outcomes and a healthier America.
